This issue examines complex disease manifestations and interventional treatment. Two contributions review extramedullary involvement in multiple myeloma and describe retrograde transarterial access for embolization of a large internal iliac artery aneurysm after previous treatment. Together, they illustrate the challenges of interpreting disease beyond its usual presentation and planning intervention in patients with altered vascular anatomy.
